Away Travel Consent Form

The below form must be completed by parents/guardians for unaccompanied U18s to travel on the coach. No completed form, no travel.

Dear parent\guardian

In order for your son\daughter to travel with Guiseley AFC Supporters’ Club to away games we need the completion of this consent form which will form part of the safeguarding procedure of the club prior to travelling.

Unaccompained U18s will also need to bring their Youth Membership card or season ticket to ensure travel on the coach, if they are yet to receive their card or season ticket a completed application will also be acceptable.

By completing this form you hereby give permission for your son\daughter named below (16 to 18 only) to travel unaccompanied on our coach and agree that your son\daughter may be photographed as part of the crowd while on a trip and these photos may be used as promotional material on behalf of Guiseley AFC Supporters’ Club.
